Integrated care systems explained: making sense of systems, places and neighbourhoods

Integrated care systems (ICSs) are partnerships that bring together providers and commissioners of NHS services across a geographical area with local authorities and other local partners to collectively plan health and care services to meet the needs of their population. The central aim of ICSs is to integrate care across different organisations and settings, joining up hospital and community-based services, physical and mental health, and health and social care. All parts of England are now covered by one of 42 ICSs.

The 15th EPH Conference in Berlin will be the first EPH Conference since Marseille 2019 to take place live and in person from 9 – 12 November. 

 

THEME AND PROGRAMME

The theme of the in person 15th EPH Conference 2022 will be ‘Strengthening health systems: improving population health and being prepared for the unexpected’. In the past years, we have learned from the Covid-19 pandemic that infectious diseases know no boundaries. Although we have seen the successful development of vaccines, we have also seen with the spread of the Delta and Omicron variants around the globe that these vaccines have not yet been available to everyone. Population health management in primary health care: a proactive approach to improve health and well-being: primary heath care policy paper series

Overview

Population health management can play a critical role in strengthening primary health care (PHC) by providing a data-driven, people-centred and proactive approach to managing the health and well-being of a defined population. By identifying subgroups with similar characteristics and needs, population health management can enable PHC providers to move from a one-size-fits-all approach to targeted and tailored interventions that account for the needs of different groups and individuals. By focusing on the social determinants of health and psychosocial needs, population health management can help PHC providers in adopting a holistic and proportionate universalism approach to address health inequalities at the community level. This publication identifies key success factors at the system, organizational and clinical levels to enable population health management in PHC. Implementation Toolbox to Document and Analyze Primary Health Care Innovations

Implementation science is a multidisciplinary field that focuses on studying and promoting effective strategies for translating evidence-based interventions (EBIs) into routine practice to improve outcomes in real-world settings. It has been increasingly used to document and assess interventions, as it helps to make sense of how, when, where, and why research results and EBI are, or are not, being successfully used. When compared to traditional project design, implementation, and management of health care interventions, implementation science can add value by addressing the specific complexities and challenges associated with implementing and scaling up these interventions. More specifically, it helps to analyze and understand the contextual factors, stakeholder dynamics, and system-level barriers that can hinder the successful adoption and integration of health care interventions.
